Parent / Guardian Mobile Device Release Form Authorization
• I understand that I may lose my mobile device privileges as a result of an inappropriate behavior, and will be financially responsible for damage or loss of any North Andover Public School Mobile Device.

• All items must be returned to the school prior to the end of the current school year.

• I understand that I will be charged for any missing or damaged equipment.

• I have read the North Andover Public Schools Acceptable Use Policy (Student Handbook) and agree to comply with the terms.

• I understand that Internet Filtering is not taking place when the device is not on our network. When the device is at home it is the parents responsibility to ensure that the student is not accessing inappropriate content.

• For iPads I must have an AppleID that is used to manage the Apps that are used on the device and I am responsible for any updates to the iPad.
